Welcome
Thank you for choosing Nourish To Flourish as your Clinical Nutritionist. These Terms & Conditions are designed to ensure a smooth and respectful experience for all patients. By booking an appointment you acknowledge that you have read and agree to these terms.
Appointments
Appointments can be booked online, in-person at Ladybird Organics or by phone.
Please ensure your contact details are current so we can send appointment reminders and communicate any changes if required.
Fees & Payments
Telehealth consults must be paid in full 48 hours prior to the appointment, an invoice with payment details will be emailed out to nominated email address and reminder messages if payment is not received. If payment has not been received 12 hours prior to the Telehealth appointment time, the consult is automatically canceled. If payment is received less than 12 hours before the appointment, it will be held for the rescheduled appointment.
In-person appointments can be paid at the time of booking via EFTPOS or cash.
Appointment prices do not include costs for supplements or functional testing.
Cancellation & No-Show Policy
We understand that unexpected situations arise. If you need to cancel or reschedule your appointment, you kindly ask that you provide at least 24 hours notice.

Cancellations with 24 hours or more notice may be rescheduled and refunded where applicable

Cancellations made with less than 24 hours' notice may incur a 50% fee.

Patients who do not attend their scheduled appointment without notifying the practice (no-show) will forfeit the full consultation fee. By booking an appointment, patients acknowldge and agree to this policy

Exceptional circumstances may be considered at the practitioner's discretion.
Nutrition Advice
Dietary recommendations are based on the information provided during your consultation. It is important that all relevant medical history, medications, supplements, allergies and health concerens are discloseed accurately.
Nutrition advice is intended to support your health and wellbeing but does not replace medical diagnosis, emergency care or treatment provided by your GP or specialist.
Individual results vary and no guarante of specific health outcomes can be made.
Supplements & Testing
Where appropriate, supplments or functional testing may be recommnded.
Patients are under no obligation to purchase supplements or complete recommended testing.
Any associated costs will be discussed before proceeding.
Privacy
Your personal and health information is colleected, stored and managed in accordance with our Privacy Policy and applicable Australian privacy legislation.
Children & Minors
Patients under the age of 18 years must be accompanied by a parent or legal guardian for their initial consultation. A parent or legal guardian mst provide informed consent before assessment or treatment can commence.
Where appropriate, older adolescents may be offered the opportunity to speak privately with the practitioner for part of the consultation. This will always be conducted in accordance with professional and ethical guidelines, with consideration given to the young person's age, maturity and best interests.
The practitioner reserves the right to determine whether a parent or guardian should be present during consultations.
Private Health Insurance
Depending on your level of cover and your health fund, you may be eligible to claim a rebate for nutrition consultations.
Patients are responsible for confirming their eligibility and rebate entitlements directly with their private health insurere prior to thheir appointment.
Payment for consultations is required in full at thee time of the appointment or booking (where applicable). Any rebate available through your private health fund is the responsibility of the patient to claim.
The practice cannot guarantee eligibility for rebates or the amount reimbursed by your private health insurer, as these vary betwen providers and individual policies.
Respectful Behaviour
Our practice is committed to providing a safe and respectful environment for patients and staff.
Abusive, threatening or inappropriate behaviour may result in termination of the practitioner-patient relationship.
